Pricing Scale and Payment Options
All patients are responsible for payment in full for each visit at the time of service. We do not file insurance or bill/send invoices here at our clinic, but we are able to provide you with the medical diagnosis billing codes and itemized receipts so that you may attempt to file a claim on your own behalf. ALL services are paid for in full the same day of service. No exceptions.
For new patients, please fill out the New Patient Registration Form form, and the Privacy Policy Form.
Initial Consultation

Follow-Up Visits

Lab Work
We suggest to all our patients to have their labs performed here at our clinic, however it is not required. Patients may elect to have labs performed at an offsite lab contracted with their specific insurance carrier, however we cannot determine whether the labs ordered by our clinic will be covered by their insurance plan.
It is strongly encouraged that all patients discuss their coverage with their insurance provider prior to any lab services rendered off-site.
